For small employers such as cafés, shops, trades and clinics who want every applicant to give the same information in the same order, and for applicants asked to "fill in an application form".

It covers the role and availability, contact details, right to work, education, the two most recent jobs and two references. It leaves out date of birth, marital status, nationality and photos: they aren't needed to judge whether someone can do the job, and many places restrict asking for them.

Questions

Can I put my company name on it?

Why is there room for only two previous jobs?

To keep it to one page. Most applicants also send a CV or résumé with their full history.

Can applicants fill it on a phone?

Is this form suitable for every country?
